Notice of privacy practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. It applies to protected health information (PHI) created or received by the licensed clinicians and pharmacy that serve patients through Vida Suya.

Our commitment

Your health information is personal. The clinicians in the LocumTele provider network and Rush Pharmacy are covered entities under HIPAA and are required by law to protect the privacy of your PHI, to give you this notice of their legal duties and privacy practices, and to follow the terms of the notice currently in effect. Vida Suya handles PHI as a business associate under written agreements that impose the same protections.

How your information may be used and disclosed

  • Treatment: to evaluate you, prescribe and monitor therapy, and coordinate with the pharmacy and other clinicians.
  • Payment: to bill and collect for services and medication.
  • Health care operations: quality review, clinician oversight, training, compliance and business management.
  • As required by law: public health reporting, adverse-event reporting to the FDA, health oversight, court orders, and to avert a serious threat to health or safety.

Uses that require your written authorization

Marketing that involves payment from a third party, any sale of PHI, and most uses of psychotherapy notes require your written authorization. You may revoke an authorization in writing at any time, except to the extent it has already been acted on.

Your rights

  • Inspect and get a copy of your medical and billing records
  • Request a correction to information you believe is inaccurate or incomplete
  • Request confidential communication by a specific means or at a specific address
  • Request a restriction on certain uses and disclosures
  • Receive an accounting of certain disclosures we have made
  • Receive a paper or electronic copy of this notice on request
  • Be notified if a breach of your unsecured PHI occurs

How to exercise a right or file a complaint

Email support@vidasuya.com and we will route your request to the appropriate privacy officer. If you believe your privacy rights have been violated, you may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/privacy/hipaa/complaints. You will never be retaliated against for filing a complaint.

Changes to this notice

We may change this notice and make the new terms effective for all PHI we maintain. The current version will always be posted on this page with its effective date.
