Telehealth informed consent

You are asked to agree to this consent before a clinician reviews your intake. It explains what telehealth is, what it can and cannot do, and the rights you keep as a patient.
What telehealth is
Telehealth is the delivery of health care using secure electronic communication rather than an in-person visit. Care through Vida Suya may be provided asynchronously — you complete a detailed written intake, and a clinician licensed in your state reviews it and responds — or by live phone or video when the clinician determines that is needed.
Who provides your care
Your clinician is independently licensed in the state where you are located and is part of the LocumTele provider network. Vida Suya does not practice medicine and does not influence clinical decisions. The clinician exercises independent medical judgment and may decline to prescribe.
Benefits
More convenient access to a licensed clinician, no travel, faster follow-up on dose adjustments and side effects, and continuity of care through the patient portal.
Risks and limitations
- A clinician cannot perform a physical examination, so a condition could be missed that an in-person visit would detect.
- Care depends entirely on the accuracy and completeness of the information you provide. Inaccurate information can lead to inappropriate treatment.
- Technical failures or delays may interrupt communication.
- Despite security measures, no electronic transmission can be guaranteed to be completely private.
- Telehealth is not appropriate for emergencies. Call 911 or go to the nearest emergency room for urgent symptoms.
- Laboratory testing, imaging, or an in-person evaluation may be required before or during treatment.
Medication and no guarantee of treatment
Completing an intake or paying a fee does not entitle you to a prescription. If a medication is prescribed, it may be a compounded product that is not FDA-approved and has not been evaluated by the FDA for safety or effectiveness. You will be told what is being prescribed, its expected effects, common and serious risks, and reasonable alternatives including doing nothing, before you agree to treatment.
Your rights
- You may ask questions at any time and expect an understandable answer.
- You may refuse or stop treatment at any time without penalty to future care.
- You may withdraw this consent in writing at any time by emailing support@vidasuya.com; withdrawal does not affect care already provided.
- You may request a copy of your medical record and expect it to be kept confidential under the Notice of Privacy Practices.
- You may request an in-person referral in your area instead of telehealth care.
Your acknowledgement
By proceeding with an intake you confirm that you are at least 18 years old, that you are physically located in a state we serve, that the information you provide is true and complete, that you have read and understood this consent, and that you agree to receive care by telehealth.
